Subscription Packages for Kaizen Medical

1. Nature of the plan
• The Kaizen Medical Skin Planner is a 12 month treatment programme paid in equal monthly instalments.
• Payments are collected by Direct Debit via GoCardless.

2. Minimum term and renewals
• The minimum term is 12 consecutive months from the date of your first payment.
• At month 12 your plan does not auto renew into another 12 month contract, unless you have served your 1 months notice. You can either restart a new 12 month plan or move back to standard pay as you go pricing.

3. What your monthly payment covers
• Your monthly payment gives you access to a set allocation of treatments, pricing privileges and priority booking that are specific to your chosen Skin Planner tier.
• The plan is personal to you and cannot be transferred to another person.

4. Cancellations and missed payments
• If you cancel your Direct Debit or a payment fails, your Skin Planner benefits will be placed on hold immediately.
• While your plan is on hold:
• Future bookings may be cancelled or rescheduled,
• Any new bookings will be charged at our standard clinic prices,
• No discounts, complimentary treatments or member pricing will apply.
• If payment is brought up to date and a new mandate is set up within 14 days, we will reactivate your Skin Planner and restore your remaining benefits for the rest of the term.

5. Early exit before 12 months
• If you choose to permanently cancel your Skin Planner before the end of the 12 month term, you will lose:
• Any unused treatment allocations,
• Any future discounts and complimentary treatments that formed part of the plan.
• For treatments you have already received under the Skin Planner at a reduced rate, Kaizen Medical reserves the right to either:
• Charge the difference between the Skin Planner rate and our standard clinic price, or
• Charge an early exit fee equal to one monthly Skin Planner payment,
whichever is lower.
• Any early exit charges will be discussed with you in advance and must be settled before any further bookings.

6. Booking rules while on a Skin Planner
• To keep your plan fair for all members, your Direct Debit must remain active and payments up to date for you to access:
• Priority booking,
• Member pricing and discounts,
• Any complimentary or bundled treatments included in your plan.
• If payments are not up to date, Kaizen Medical may cancel or amend future bookings and revert all appointments to standard pricing until payment is resolved.

7. Pausing your plan (exceptional circumstances)
• In certain medical or exceptional circumstances, we may agree to pause your plan for a defined period.
• Any pause is at the clinic’s discretion and will be confirmed in writing.
• During a pause, payments and benefits are both suspended and the end date of your 12 month term may be adjusted.

8. Changes to pricing or terms
• We may review Skin Planner pricing or inclusions from time to time.
• Any changes to your existing plan will be communicated with reasonable notice. You will have the choice to continue under the new terms at your next renewal or revert to standard pricing.

9. General
• All treatments are subject to medical assessment and suitability.
• Kaizen Medical reserves the right to decline or discontinue treatment where it is not clinically appropriate, even if it is included within your plan. In such cases we will discuss alternatives with you.
